Password resetter MOD -- question

I have added the password resetter mod...but in the README file, it mentions that the code to add in index.php occurs 3X.  I can only find it occuring 2X.

It occurs three times in 1.3.x.

you can also try searching for $txt[14], it occurs three times also and that will put you exactly where you need to be (or right in the middle of what you are looking for)
